Did this work for you? My sister tried it, but they just asked for an email address and said it was on the way. Didn't tell them her address or anything like that.
It just asked for my birthday, then said the sample was on it's way. I've requested samples from this number before, so I'm thinking they have my information on file, attached to my phone number. Just a guess!
